Appointments can be made either by telephone, in person or via Emis Access (see information tab). The practice offers 10 minute appointment slots for routine matters. Each slot is for one person only and you are encouraged to see the same doctor for the same problem, if possible.

Routine surgeries generally run from 08:30 - 11:30 and from 14:00 - 16:30

Extended hours appointments are intended for people who find it difficult to attend during normal working hours. These are usually held from 17:30-19:00pm on a Monday.

Our aim is to offer you an appointment with the doctor of your choice within one week, except when that doctor is on annual leave.

Please tell reception staff if you need to be seen urgently as the practice operates a triage system. The on-call doctor will contact you and assess whether you need to be seen the same day.

If you cannot keep an appointment please inform the practice as soon as possible, ideally giving 24 hours notice.

Test Results

tests_and_results Please call between 11am and 3.30pm to enquire about your test results as our reception staff will have more time to deal with your request between these times.

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.

When you take your test you will be told how long it will be before the results are returned to the practice. It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so.

doctors_bag Home visits are reserved for those who are housebound or too ill to attend the surgery and will be made at the doctor’s discretion.

Patients are asked to make their request, where possible, before 10.00 am.

Facilities for examination and treatment are superior at the Health Centre and, as it is located centrally, you will generally be seen earlier if you can attend.

A doctor is available at the Practice for advice on emergencies between 8.00 am and 6.00 pm. It is always better to contact the surgery by telephone, if possible, before presenting at the surgery. If possible try to call surgery between 8am and 10am and use the emergency number.

Emergencies – outwith normal working hours (Tel: 08454 24 24 24)

NHS 24 covers all emergency calls outwith the surgery opening hours. A trained nurse will answer your call and make arrangements for you to be seen by a doctor, if necessary.

Ease of use

EMIS Access is an appointment booking service which has been carefully designed for ease of use, and can be used by anyone who has a computer, mobile phone or digital TV and is being developed in partnership with NHS 24 and Looking Local.

It’s quick and easy to use, and can be particularly useful during those times when the practice is closed, telephone lines are busy or if you are away from home. The easy to follow screens and prompts ensure it’s simple for you to view, book, or cancel appointments online – whatever time of the day or night.

Using EMIS Access, you will be able to view a range of available appointments and choose the most convenient time for you.

Secure information

All personal information contained on EMIS Access is protected using the highest standard internet security. Before you can start booking appointments online, you must first register your details for this service.

It’s quick and easy to register – simply follow the three steps below.

Remember, when using EMIS Access all your personal information is safe and secure.

Registering is three easy steps

Step 1. Ask at reception for a registration form. You will need to bring in photographic ID to show reception staff, purely for confidentiality purposes. This provides all the information you need to register to use the EMIS Access facilities.

Step 2. Choose the media that you want to use.

Other options include using the internet, go to the practice website www.polmontparkmedicalgroup.co.uk and click on the link.

Or downloading an app from itunes if you have an iphone and the android app should be available soon.

Search for the free looking local app or text your postcode to 61061, your network charge with apply for text.

Step 3.

Create your account.

Depending on the media used the screens will be different. Go through the screens selecting the appropriate options, then select NHS Scotland and NHS 24 this will take you to GP Services . Select use GP Services and enter your personal registration details exactly as they appear on the registration form. If the information does not match, your registration with not be accepted.

You are now ready to book and cancel appointments.

Once registered, simply log on with your practice ID, patient ID and password. You can view any appointments you have already made, cancel them if necessary, or book a new appointment after checking which times are available.

There is no need to re-confirm by email or telephone – just arrive promptly on the day.

Looking Local is a national public sector service available on digital interactive TV (Sky, Virgin Media, broadband enabled Freeview), internet enabled mobile phones, smartphones and consumer devices such as the Nintendo Wii, all of which appeal to people who do not use the Internet, as well as complementing websites for PC veterans who want to undertake a quick and easy request or find information/services on the go. A former National Project now owned by Kirklees Council on a not for profit basis has many thousands of people every week, using their TV and mobile to get information. This engagement is resulting in more efficient service provision but also better access to services when people want it. Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors, please telephone or use EMIS access.

If you wish to cancel a Practice Nurse appointment please telephone or present at the reception desk.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.
Change of personal details
Please inform the reception staff if you change your personal details such as name, address, telephone number or email address, so that your records can be amended.
If your address falls outwith the practice area you will be required to register with another doctor in the new area.
