Within ForthValley there are various services and charity organisations available to patients/carer’s and next of kin with learning disabilities.  We hope this leaflet will let you know a bit more about them and what we offer in the Practice.

 

SCLD

The Scottish Consortium for Learning Disability is a charity that is made up of 12 Partner Organisations who have joined together with funding from the Scottish Government to help people make the changes set out in 'The same as you?'

‘Our goal is an inclusive Scotland where everyone is valued and respected for who they are and what they contribute as equal citizens.’

PAMIS

In partnership with people with profound learning disabilities and their carers.   This charity has a wealth of knowledge on their website from;

Autism Alert Card

The card enables those who carry it to identify themselves as having an autism spectrum disorder.  The card has a named contact on the back who can offer their support to help public and emergency services professionals understand the individual’s autism needs.

Your Practice – Polmont Park Medical Group

We have all the information and links included in this leaflet available on our website www.polmontparkmedicalgroup.co.uk along with other health information/services and contacts that may be of use.  The Practice offers an online repeat prescription service and an online appointment booking option to see a doctor.  For more information visit the website or ask at reception. 

Within the Health Centre there are ‘Well man/woman clinics’ that run in the evenings.  The clinics offer full health checks and all details will be passed on the doctor and recorded in your records.  To enquire about these, or to book an appointment, please call 01324 717920.

We hope that this information has been of use to you and if you know of  other services that may be of help to others, please do not hesitate to contact the Practice Manager, Jill Pate on 01324 715753.

We have also enclosed a ‘medical information sheet’ that may be useful.  This could be brought to future consultations with a doctor or nurse.  These will be available to download from our website under the ‘learning disabilities’ tab at the right hand side of the screen.

    Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors, please telephone or use EMIS access.

If you wish to cancel a Practice Nurse appointment please telephone or present at the reception desk.


By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.
Change of personal details
Please inform the reception staff if you change your personal details such as name, address, telephone number or email address, so that your records can be amended.
If your address falls outwith the practice area you will be required to register with another doctor in the new area.
